What is Community Supports (CS)

What community supports are available?

Short-Term Post-Hospitalization Community Supports (CS) provides temporary housing and supportive services for members who do not have a stable residence and have significant medical, behavioral health, or substance use disorder recovery needs. The service is intended to support members immediately following discharge from an inpatient hospital, residential substance use disorder treatment or recovery facility, residential mental health treatment facility, correctional facility, nursing facility, or recuperative care program.

Recuperative Care also known as Medical Respite Care, is a Community Supports service designed for individuals who are experiencing homelessness or are at risk of homelessness and require a short-term residential setting to safely recover from an acute illness, injury, or behavioral health condition.

The program provides a safe environment that supports recovery, promotes health stabilization, and reduces avoidable utilization of emergency and inpatient healthcare services.

Housing Transition Navigation Community Supports (CS) assists members experiencing homelessness in obtaining and securing permanent housing. The service provides individualized support throughout the housing search and placement process, including assistance with completing housing applications, benefits advocacy, identifying and securing available housing resources, and coordinating with landlords and property managers.

Housing Transition Navigation services are designed to address barriers to housing stability by helping members access community resources, navigate housing systems, and successfully transition into permanent housing. The goal is to promote long-term housing stability, improve health outcomes, and reduce the risk of homelessness and unnecessary utilization of healthcare services.
